Your UHC medical ID card

After you enroll in a medical plan, your new ID cards will arrive at your home address on file in about seven to 10 business days. You’ll receive two cards.

You should also create your account at myuhc.com. Once you’re signed in, you can:

• Print extra ID cards for yourself and your family.
• View a digital version of your ID card on the UHC Mobile App (search for “HEALTH4ME” in your app store).
• Send your card directly to your doctor’s office from the app.
• Find providers, check your claims and more.

Your Kaiser medical ID card

Your Kaiser ID card will be mailed to you if you’re a new member, have had a name or birth date change or request a new card. Use the Kaiser Permanente app for quick access a digital version of your ID card. Use the app to check in for appointments, pick up prescriptions and show proof of membership at any Kaiser location.
