How to report a leave of absence
You may apply for a leave of absence within 30 days of the tentative start date of your leave. Please give your supervisor as much notice as possible.
To apply, you’ll need to create an account with Reliance Matrix.
Once you create an account, you can apply for disability:
- Online: Sign in to the Reliance Matrix website
- Mobile app: Download and sign in to the Matrix Mobile App (App Store | Google Play)
- Phone: Call 1-888-288-1114, available 24/7
You’ll receive a packet within 24 hours of applying. It will explain what documentation, if any, is required to process your claim.
